Are you an experienced Registered Social Worker with a passion for improving patient journeys through acute mental health services? We have an exciting opportunity to join our Tiaho Mai team at Middlemore Hospital as a Transitions and Complex Care Coordinator.

This newly established role is central to ensuring seamless transitions into and out of our inpatient mental health service, supporting timely admissions, reducing discharge delays, and coordinating care for tangata whaiora with complex needs.

About the role

Working within Tiaho Mai Acute Inpatient Mental Health Services, you will partner closely with Acute Community Services, multidisciplinary teams, NGOs, housing providers, and whaanau to facilitate safe, coordinated, and recovery-focused care.

You will:

  • Coordinate admissions, transitions, and discharge planning across inpatient and community services.
  • Identify and address barriers that impact patient flow and recovery.
  • Lead multidisciplinary and multi-agency care planning meetings for complex cases.
  • Build strong partnerships with internal and external services to support equitable, person-centred outcomes.
  • Monitor complex cases and contribute to service improvement initiatives that strengthen patient flow and system performance.

About you

  • Registration as a Social Worker with a current Annual Practising Certificate.
  • At least five years' post-registration experience.
  • Extensive experience supporting people with complex mental health and social needs.
  • Strong knowledge of discharge planning, community pathways, and multidisciplinary practice.
  • Excellent communication, negotiation, and relationship-building skills.
  • A commitment to Te Tiriti o Waitangi and achieving equitable health outcomes for Māori and all communities.
  • A full driver's license.
