About the Team/Service
SPARC is a multidisciplinary team comprising a Clinical Nurse Specialist, a community-based Geriatrician and a Pharmacist. The team provides specialist care for older people living in aged residential care, working collaboratively with primary care teams to identify acute deterioration early and coordinate appropriate treatment and follow-up plans.
The service aims to support residents to remain in aged residential care where appropriate, avoiding unnecessary transfers to the Emergency Department or admission to hospital. The team also provides education and training to aged residential care clinical staff on geriatric and frailty-related conditions.
