About the Community Occupational Therapist Role
This role is community-based and centres on:
The community rehabilitation service supports people with neurological, long-term, or age-related conditions, including stroke, dementia, frailty, and post-trauma. The multidisciplinary team includes Physiotherapists, Social Workers, Dietitians, Speech Language Therapists, Nurse Practitioners, Allied Health Assistants, and a Geriatrician.

- Providing rehabilitation solutions to maximise independence and achieve functional goals
- Working collaboratively across disciplines to ensure smooth, person-centred care
- Developing your skills and career with scope for ongoing professional development
The community rehabilitation service supports people with neurological, long-term, or age-related conditions, including stroke, dementia, frailty, and post-trauma. The multidisciplinary team includes Physiotherapists, Social Workers, Dietitians, Speech Language Therapists, Nurse Practitioners, Allied Health Assistants, and a Geriatrician.
