Goodwood Park Healthcare Group is a progressive rehabilitation-based organisation and a leading provider of residential and community support services for people who have a Mental Health illness, Traumatic Brain Injury, Intellectual Disability, or dual diagnosis.
Currently, we are looking for a Registered Health Professional (OT, Registered Nurse, Drug & Alcohol Practitioner, Social Worker) with an APC/registration to provide clinical services to our clients based at our residential sites in West and South Auckland.
The role:
Following an assertive case management framework, you will provide client-centred clinical support and therapeutic rehabilitation to clients to ensure their independence is maximised/maintained, and recovery is optimised to enhance their quality of life and sense of well-being.
The core tasks of this role include:
· Conducting clinical assessments to evidence outcome measures
· Formulating and implementing high quality client rehabilitation plans in collaboration with the multidisciplinary team
· Delivering individual and group therapeutic interventions as clinical keyworker
· Working collaboratively with clients, their families and other stakeholders
· Providing integrated care with other community organisations or service providers
· Meeting contractual KPI’s,
· Participation in multi-disciplinary meetings within the organisation and case reviews with funders or stakeholders
· Coaching and supporting Rehabilitation staff with goal planning and intervention
· Maintaining relationships with Mental Health clinical services, other health services as appropriate for the clients and their health needs, and ACC case managers
Our ideal Registered Health Professional for this role will possess the following skills and attributes:
· A passion for mental health/TBI/Intellectual Disability rehabilitation
· Driven to foster positive change in clients
· Confident with report writing, good time management and great organisational skills
· Able to work to deadlines, demonstrate resilience and maintain a positive attitude
· An Annual Practicing Certificate or registration
· Two years’ experience working in the disability and rehabilitation fields
· Team oriented and a great communicator
· A full NZ drivers licence
· Proficient computer literacy – Microsoft suite
· NZ residency
· Due to the 24/7 nature of our services, some evening/weekend work may be required in this role, and this role forms part of the oncall roster for the organisation
