This is a 0.8 FTE Health Coach flex role, combining Health Coaching and Support Work within the Hornby community in Ōtautahi. Employed by Purapura Whetu Trust under the Te Tumu Waiora initiative, the role supports individuals on their haerenga toward improved health and wellbeing. We seek a proactive kaimahi passionate about empowering people through health literacy, motivation, and personalised support grounded in manaakitanga and whanaungatanga.

 Job Description

Key responsibilities include:
• Conducting initial assessments to understand whaiora health needs and personal goals.
• Developing tailored health and wellness plans that align with whaiora needs and aspirations and goals.
• Providing ongoing coaching sessions to guide whaiora through the implementation of their health plans.
• Supporting whaiora with goals for nutrition, management of physical activity, stress management, and other lifestyle factors affecting their health.
• Offering motivational support to help whaiora overcome barriers and stay committed to their health journeys.
• Collaborating with other healthcare professionals within the GP practice to ensure comprehensive care for whaiora.
• Maintaining accurate records of client interactions, progress, and feedback for continuous improvement.

 Desired Skills and Experience

Required skills
• Excellent communication and interpersonal skills
• Strong understanding of health and wellness principles
• Ability to motivate and inspire whaiora towards their health goals
• Knowledge of mental health and addiction,  nutrition, fitness, and lifestyle management.
• Strong organisational and time management abilities
• Competence in using IT
• Commitment to working in a culturally responsive, Te Ao Māori-informed models of care

Required experiences
• A minimum of level 4 Health and Well-being, mental health, and addictions
• Experience working with diverse populations and understanding their unique needs
• Previous involvement in developing workshops or group sessions on health topics
• Experience collaborating with healthcare professionals to support whaiora outcomes
