Pharmacist (Clinical Pharmacist)

Location: Ōtara & Manurewa Hours: Full-Time | Fixed-Term (6 Months, Maternity Leave Cover)

About the Role

The Wellness Support Team is seeking a Pharmacist to join our award-winning team on a fixed-term basis to cover maternity leave.

This is an exciting opportunity to work within an innovative primary care setting, supporting patients and whānau to achieve better health outcomes through safe and effective medication management. Working alongside General Practitioners, Nurses, Health Improvement Practitioners, Health Coaches, and other healthcare professionals, you will play a key role in improving access to medicines, reducing health inequities, and supporting the wellbeing of diverse South Auckland communities.

Based across our Ōtara and Manurewa clinics, you will provide clinical pharmacy services within a multidisciplinary model of care, helping patients better understand and manage their medicines while supporting healthcare teams to optimise treatment outcomes.

What You Will Do

  • Optimise medication use to improve patient health outcomes and safety.
  • Undertake comprehensive medication reviews and clinical evaluations.
  • Support medication reconciliation and safe prescribing practices.
  • Provide medicines education and support for patients and whānau.
  • Improve medication adherence and awareness of medicine-related risks.
  • Support the management of long-term conditions through clinical recommendations and prescribing where appropriate.
  • Work collaboratively with General Practitioners, Nurses, and other healthcare professionals to deliver coordinated care.
  • Contribute to quality improvement initiatives, medication audits, and service development activities.
  • Promote culturally safe practice and equitable health outcomes.

What You Will Bring

  • A Bachelor of Pharmacy or equivalent recognised pharmacy qualification.
  • A current Annual Practising Certificate without restrictions.
  • A postgraduate clinical pharmacy qualification, or be working towards one.
  • A strong understanding of Te Tiriti o Waitangi and its application within healthcare.
  • Knowledge of health inequities relating to medication access and use in Aotearoa New Zealand.
  • Excellent communication, organisational, and critical thinking skills.
  • The ability to build strong relationships with patients, whānau, and healthcare professionals.
  • A commitment to culturally safe, patient-centred care.

The following would be advantageous:

  • At least two years of recent clinical experience within primary care or hospital pharmacy.
  • Experience working within multidisciplinary healthcare teams.
  • Experience providing clinical advice and support to other healthcare professionals.
  • Experience working with diverse Māori, Pacific, and multicultural communities.

About the Wellness Support Team

Established in 2012, the Wellness Support Team provides integrated health and wellbeing support within primary care settings across Auckland. Our multidisciplinary team includes Pharmacists, Health Improvement Practitioners, Health Coaches, Therapists, Support Workers, and Clinical Leaders who share a commitment to accessible, person-centred care.

The programme has been recognised nationally, receiving the award for Best Mental Health Programme at the New Zealand Primary Healthcare Awards. Through a collaborative and innovative approach, we help ensure patients and whānau receive the right support at the right time.
