Clinical Nurse Specialist - Primary Care Liaison

The Primary Care Liaison role will be a Clinical Nurse Specialist, that will have the responsibility of coordinating and supporting primary care with shared care and communication pathways within the primary/secondary interface. Focus will be on GP and schools.

The role is pivotal in the on-going development of an integrated service between primary and secondary mental health care with a focus on creating a more responsive service to children and adolescents referred for primary mental health care with mild to complex issues.

This is a stimulating, challenging and autonomous position to provide clinical leadership and provide service improvement to meet the challenges of future mental health care delivery across the CY&F MHS portfolio.

We are looking for a Clinical Nurse Specialist with:

  • Significant experience in a variety of mental health clinical backgrounds.
  • Excellent communication and networking skills with service users, family/whanau, team members, other agencies
  • Established leadership skills
  • The ability to prioritise urgency of response and level of response required.
  • Skills in sharing knowledge, including delivery of teaching in primary care settings
  • Innovation, organisation and flexibility
