About the Role

Play an important role in connecting tamariki and whānau with the services best placed to support them. As part of the Child Health Integrated Response Pathway (CHIRP), you will coordinate referrals for children with developmental, attentional, and behavioural concerns, gathering and interpreting information to help determine the most appropriate pathway. Strong relationships with whānau, health professionals, education, and community services are central to providing timely and well-coordinated access to care.

About the Team/Service/Location

CHIRP works alongside Child Development Services, Voyagers, and Paediatrics, bringing multidisciplinary expertise together to support tamariki aged 14 years and younger. Working across health, education, Māori health, and community services, the team provides a collaborative pathway that helps whānau navigate specialist and community support with greater clarity and confidence.

About You

You are an experienced health professional who enjoys bringing information together, making sense of complexity, and finding the right pathway forward. Thoughtful, organised, and confident working autonomously, you build trusted relationships and keep tamariki and whānau at the centre of your decisions.

What you will bring:

  • Current registration and Annual Practicing Certificate as a Registered Practitioner
  • Experience working within child development and/or behavioural health, with knowledge of mental health
  • Experience working within a healthcare environment
  • Strong clinical reasoning and the ability to analyse information from multiple sources
  • Confidence coordinating referrals, prioritising workloads, and working autonomously
  • Strong communication and relationship-building skills across health, education, whānau, and community services
  • Confidence using clinical and patient management systems
  • A collaborative approach and commitment to continuous service improvement
