An exciting opportunity has become available for an experienced and motivated Community Clinical Nurse (CCN) to join the Child Health Community Service within Pa Harakeke: Healthy Women, Children, Youth and Community.

Our Child Health Community Service is a multidisciplinary, community-focused team that works across the paediatric continuum to deliver high-quality, whānau-centred care. As a Community Clinical Nurse, you will work closely with nursing, social work, and other health professionals to support children, young people, and their families across the MidCentral rohe.

The Community Clinical Nurse (CCN) plays a key role in improving health outcomes for children and their whānau across the MidCentral rohe.

A central component of the role is the delivery of nurse-led clinics for children with eczema and asthma, providing specialist assessment, education, treatment, and ongoing support. Working in partnership with children, families, and whānau, you will identify and address the health and social factors that influence wellbeing, ensuring care is holistic, equitable, and family-centred.

You will work across the paediatric continuum in both primary and secondary care settings, building capability within the workforce, strengthening care pathways, and promoting evidence-based practice.

Key responsibilities include:

• Delivering nurse-led eczema and asthma clinics.
• Providing specialist assessment, intervention, education, and follow-up care.
• Supporting children and whānau with complex health and social needs.
• Building clinical capability across primary and secondary care.
• Collaborating with multidisciplinary teams and community partners to improve outcomes for children and young people.

To be successful in this role, you will be an experienced registered nurse with a commitment to delivering high-quality, whānau-centred care and improving outcomes for children and young people.

You will have:

• Current Nursing Council of New Zealand registration and a valid Annual Practising Certificate.
• A minimum of five years' experience as a Registered Nurse.
• Demonstrated experience in child health or paediatric nursing preferable.
• Proficient clinical assessment and decision-making skills.
• A commitment to culturally safe, equitable, and family-centred care.
• Strong communication and collaboration skills.

Our team is a multidisciplinary team that is community facing but works across the paediatric continuum. The Community Clinical Nurse role will work alongside the other nursing and social work members of the Child Health Community team.
