We are seeking two motivated and adaptable Continuation Support Officers to join our Facilitated Support team on a temporary basis. These roles are designed to create additional capacity within the team, enabling our Customer Service Representatives to undertake further training and development.

While the primary focus will be on continuation work, these roles are flexible and will also include client-facing interactions, scheduling and bookings, and a variety of administrative tasks.

Key Responsibilities

  • Complete file reviews by assessing existing client/client representative information and identifying person-centred, individualised outcomes based on current and future needs
  • Review file notes and communications to determine whether clients require formal reassessment and ensure support plans remain up to date
  • Liaise and communicate with providers, clients, and external stakeholders while maintaining accurate data entry across systems
  • Escalate tasks in line with Priority Framework guidelines to support timely and appropriate service delivery
  • Coordinate and schedule assessments for new and existing clients/client representatives across multiple services
  • Ensure bookings are managed according to priority frameworks, escalation processes, and urgent/crisis assessment requirements
  • Collaborate closely with Support Facilitators, Specialists, and Pathways teams to effectively allocate booking availability and manage cancellations
  • Communicate directly with clients/client representatives to arrange assessment dates, times, and suitable assessment methods including virtual, face-to-face, or in-home appointments

About You

  • Experience dealing with customer face to face and over the phone and emails
  • Strong communicators with the ability to manage expectations clearly
  • Highly organised with excellent attention to detail
  • Comfortable using systems such as Outlook, Word, Excel, and Socrates
  • Able to make sound, timely decisions within guidelines
  • Flexible, adaptable, and willing to take on a variety of tasks
  • Comfortable working in a fast-paced, evolving environment
  • Experince in Health or disability support environment
